What Is a Self-Reflection Form?

A self-reflection form gives employees space to share their perspective on their work over a specific period of time. It also helps managers understand whether their view aligns with the employee’s experience.

You can use responses from the form to set future goals, identify areas for growth, and surface potential misalignments early—before they turn into larger issues.

What Should a Self-Reflection Form Include?

A self-reflection form typically includes the following elements: basic employee information, skill and performance reflections, accomplishments, and areas for growth.

You can combine open-ended questions with structured sections to give employees room to share context while still gathering consistent, comparable input.

Employee Information & Grading Scale

Your form should include:

  • Basic employee information, such as name, role, and department

  • The time period being reflected on (often since the last review or check-in)

  • A clear rating or confidence scale, if applicable

If you choose to include a scale, it can be numerical or descriptive—as long as it’s clear what each level represents. A common approach is a 1–5 scale, where 1 represents “needs improvement” and 5 represents “strong performance.”

Skills & Performance Reviews

The form should include sections that prompt employees to reflect on key skills and performance areas relevant to their role. List the skills that matter most—anything from technical expertise to communication or leadership—and invite employees to reflect on their strengths and challenges.

You can also include prompts related to performance indicators, such as meeting deadlines, collaborating with others, or managing responsibilities over the given time period.

Areas for Improvement

Under each skill or performance indicator in the list, you can allow the employee to make suggestions. Let them show how they think they can improve their performance and what actionable steps they can take. This can help management gauge an employee's objectivity and motivation. Further, the suggestions can help management encourage professional development and optimize the employee’s skill set.

Accomplishments & Goals

Finally, include an open-ended section where employees can highlight accomplishments, challenges they’ve worked through, and progress they’ve made since the last check-in. Encourage them to share their goals for the future as well.

Understanding both strengths and aspirations helps managers plan next steps, support development, and make better use of each employee’s skills.
