Create a WordPress Form in Minutes

Learn how to use our quick and easy form maker plugin to create a form and share it on your WordPress site in minutes.

1. Create a form

2. Install the Opinion Stage WordPress Form Plugin

Go to the WordPress plugin library, search for the Poll, Survey & Quiz Maker Plugin by Opinion Stage, and install it.

Next, activate the plugin.
Note: If you prefer, you can download the plugin and upload it manually.

3. Connect Opinion Stage to the WordPress Plugin

In your WordPress dashboard, select Opinion Stage. Then log in to your account or sign up if you haven’t created one yet.

4. Embed your form on your WordPress site

You’re ready to embed your form. In Gutenberg, click the + button to open the block menu, then search for the Opinion Stage Form/Survey block.

Click Select Item to browse your existing forms in the Opinion Stage items library. If you still need to create one, choose Create a Form/Survey to get started.

If you’re using the classic editor, click the Add a Poll, Survey, Quiz, or Form button above the editor to open the form insertion window.

5. Save your work

Now all you have to do is save your page/post.

Here are some tips and suggestions that will help you take your form to the next level.

Keep It Short

Include only the must-have fields, remember that the more fields you have in the form, the fewer submissions you will get.

Customize the Look & Feel

Customize the form so that it fits your site and brand.

Get Notified

When relevant, configure to get notified each time a user submits information in the form so you can act on it in real-time.

Integrate It

Integrate the form with your marketing tools to automatically insert the gathered information into your tools. For example, if you are gathering emails, you can automatically send them to your email-sending service (e.g. MailChimp) with all the information captured.
